IF SELF PAY:

Initial intakes take approximately 60 minutes for medication management: $300

Follow up appointments are 30 minutes and include therapeutic modalities and management: $150

 

BEFORE BEING SEEN:

Clients need to sign the following forms, provide current insurance information, complete clinical history forms on the portal and a copy of their driver's license
