Self-Pay Rates:

Initial Intake: $275

Initial intakes take approximately 60 minutes for medication management​

Follow Up: $150

Follow up appointments are 30 minutes and include therapeutic modalities and management

BEFORE BEING SEEN...​

Clients will need to have set up their MYIO Patient Portal (sent from Monarch) and completed the questionnaires. These questionnaires / clinical history forms are sent directly to your provider and required for your initial intake appointment. Clients will also need to provide current insurance information, a copy of their driver's license, and complete our office intake consent forms (please see "Intake / Onboarding Process" in FAQs).
